Where can I find a copy of our Patient Agreement?

There are two ways that you can find this document:

1. If you have been with us before January 01, 2024 you can view the document following these steps: 

• Log into your BoomCloud Dashboard via membership.boomcloudapps.com
  
  1. Click on the Settings Button  in the upper right hand corner of the screen.
  2. Select the "Legal Settings" Tab
  3. Once you have done so, you will be able to view the document and it should look like this: 
     
      
     

 

2. If you are an office that has started with us on January 01, 2024 and beyond, you can view the document following these steps:
